We have a social work vacancy in our assessment team who will have the main responsibility for working with our health colleagues to restart Continuing Health Care Assessments (CHC).

 

About the Roles

The Council is committed to embedding a ‘strengths-based approach’ and delivering social care by implementing exciting new initiatives ‘locality based models of care’ and in partnership with Health and Trust colleagues and community and voluntary sector partners.

All social care staff in the council are expected to lead in applying the Care Act, Mental Capacity Act and social care agenda to safeguard Haringey residents. All social care staff are experts in championing the Council and local community resources, have excellent liaison and joint working skills and understand the importance of a holistic approach to the health and social care needs of residents.

Your responsibilities will include taking part in team referrals, duty, attending Perform+ huddles, Council forums, and liaising with other adult teams, Council services like Housing, Brokerage, Reablement, MH Trust teams, CCG’s, care homes and inpatient wards to support people’s health and social care needs and ensure that residents are safeguarded.

You will be confident in all aspects of social care legislation and the community resources people can use or be referred to in Haringey. You will be working in one of London’s liveliest and most culturally diverse Boroughs. The Council is signed up to the use of forums such as Community Care Inform and tools such as Tri.x and Perform+ and other national professional development networks to support you in delivering excellent outcomes for residents and support the learning and development of social care staff in Haringey.

You will be confident in accessing and using Council resources and systems including MOSAIC, Hari-NET, and Connected Communities. 


About You

You will be an experienced practitioner with a good understanding of CHC processes, able to work effectively within an MDT to facilitate completion of the CHC checklist and Decision Support Tool (DST) where appropriate. You will have previous experience of working in a multi-disciplinary team, including that of health and CHC nurse assessors. You will have a sound knowledge of legislation that underpins health and social care for adults. You will be aware of good practice guidance from organisations such as ADASS, Social Work England, and CQC. You will have good IT skills and a competent approach to the use of health and social care records, processes and systems to evidence your work. You will embrace and model the Haringey values in your day to day work.

Other Requirements

These posts are exempt from the Rehabilitation of Offenders Act 1974. An enhanced disclosure check with the DBS will be undertaken.
