Transition Social Worker
An exciting opportunity has arisen within the Haringey Community Learning Disabilities Team to join the Transitions Pathway as a Transitions Social Worker.
You will be responsible for managing a range of cases for young people, supporting them and their families throughout their transition into adulthood. The responsibilities will include joint working with Children’s Services and SEN, developing Transition support plans for young people, undertaking reviews, completing carer's and adult needs assessments, attending LAC review meetings, carrying out Safeguarding investigations and attending relevant meetings with partner agencies and attending court hearings.
About you
We would like to hear from candidates who:
- Have a relevant professional qualification in Health or Social Care
- Have previous knowledge and experience of the needs of children and young people with learning disabilities and relevant legislation
- Can test and determine eligibility criteria for services for people with Learning Disabilities
- Are able to work positively as a member of a multi-disciplinary team on complex cases and work positively with young people with their families to ensure positive outcomes are achieved as a result of in to adulthood
Please note this post is being offered as a fixed term 12 month contract. Options for part time and flexible working are available.
