The Housing Related Support (HRS) Service has been working using the Making Every Adult Matter (MEAM) approach since 2019. This approach seeks to deliver effective, coordinated services that directly improves the lives of people facing multiple disadvantages, with coproduction at the heart. 

This is an opportunity to be part of the operational delivery of the MEAM approach in Haringey. This will involve offering a flexible and creative approach to those for whom traditional support methods have not worked. Providing longer term, transitional support, and interventions in the community, holding a caseload of up to 15 individuals. 

You will coordinate professionals working with your caseload, carry out joint assessments; facilitate the sustainment of accommodation, health and social/personal outcomes using a strengths-based approach. You will work with 

You will also be working as part of a multidisciplinary team of housing, health, and social care partners to tackle the harms and inequalities faced by people who sleep rough. You will contribute data, insights, and coordination support into the MEAM partnership and the Rough Sleeping Taskforce.

About you 

We are looking to recruit motivated, creative and independent Intensive Support Navigators to lead the operational delivery of the MEAM approach in Haringey. As the post-holder, you will be working in partnership with a range of housing, health, and community organisations to ensure service users receive personalised interventions, wrap around support to move forward with their lives.

We are particularly interested in hearing from people with personal experience of homelessness, mental health, substance use or criminal justice involvement. We also encourage applications from Black and Minority Ethnic and transgender people who are over-represented in homelessness services and under-represented in the workforce.
